There are two common causes for this issue:
1. Sensors are not detecting the controllers
- Make sure your hands are within the play area
- Check that the glossy side of the sensors faces the play area
- Remove any obstructions blocking the sensors
2. Error in the Oculus app
- Restart your PC
- Launch the Oculus app
- Check the status of sensors and controllers in the Devices menu
3. The the batteries in the controllers are nearly depleted.
- Remove the old batteries
- Replace with new batteries
- Test controllers to ensure they are functioning properly
